Study aim
1. Effect of N-acetyl cysteine on the prevalence of acute kidney injury in children admitted to PICU 2. Effect of Q10 on the prevalence of acute kidney injury in children admitted to PICU 3. Comparison of the effect of N-acetyl cysteine and Q10 on reducing the prevalence of acute kidney injury in children admitted to PICU
Design
A randomized, double blinded clinical trial study with community-based and pragmatic control group, and parallel groups
Settings and conduct
Before the study, a questionnaire will be prepared which contains data on demographic information of the patients (including age, sex, weight and height, and etc.). After patients selection based on inclusion and exclusion criteria, the initial concentrations serum of Cr and BUN in all patients will be recorded and evaluated. Patients will be randomly divided into 3 groups, including NAC (n=30), Q10 (n=30), and placebo (n=30). During the survey, serum levels of Cr and BUN will be measured and evaluated periodically based on RIFLE criteria. The other vital parameters such as blood pressure, weight of patients, CBC, and electrolytes will be carefully considered and controlled.
Participants/Inclusion and exclusion criteria
children without history of kidney disease and getting minimum score 10 from PRISM 3 scoring table will be selected for this survey. Children who have chronic kidney diseases will be excluded from the study. Furthermore, child's who have to use some types of drugs which have interference with the study drugs will be excluded from the survey. children shouldn’t participate in another study at a same time and they will be excluded in case they present sensitivity to the study drugs.

In this survey, children's parents know about the study drugs, but they will not be informed about grouping. The grouping will be clarified with A, B and C, and drugs will not be identified by all persons except researcher and disciplinary committee for data supervision.

Primary outcomes

Description
The important outcome of the current survey is the effect of NAC and Q10 in prevention of AKI which is performed by measuring several factors such as urine volume, blood urea nitrogen, creatinine, hemoglobin, white blood cells, platelet, sodium, potassium, phosphor, calcium and magnesium elements, as well as consideration of glomerular filtration in the kidney.
Timepoint
Patients in NAC group will receive 8mg/kg/TDS NAC and patients in Q10 will be treated with 3 mg/kg/day Q10 during admission time at PICU.
Method of measurement
Blood urea , creatinine, hemoglobin, white blood cells, sodium, potassium, will be measured using ELISA method. AKI disease severity will be evaluated using RIFLE standard criterion.
