Study aim
To assess the effect of self-stretching on the clinical and electrophysiology symptoms of patients with carpal tunnel syndrome
Design
This is a Phase III randomized clinical trial with a control group with parallel groups, in which eligible patients will be randomly assigned to the intervention and control groups using block randomization.
Settings and conduct
This study will be conducted at the Besat Hospital in Hamadan city, involving 96 eligible patients with carpal tunnel syndrome. The patients will be randomly assigned to the intervention and control groups through the block randomization. However, it should be noted that blinding is not feasible in this trial.
Participants/Inclusion and exclusion criteria
Inclusion criteria: Age 18 to 90 years Carpal tunnel syndrome Exclusion criteria: Neuropathic disorders History of surgery or trauma to the wrist
Intervention groups
Intervention group: Routine treatment plus self-stretching exercises at home for 30 seconds 4 times a day for 6 weeks Control group: Routine treatment plus sham stretching movement at home for 30 seconds 4 times a day for 6 weeks
Main outcome variables
Primary outcome: Average pain intensity, sensory disorder of the fingers, amplitude and sensory and motor latency of the median nerve, clinical symptoms

Randomization description
In this study, random assignment of patients to the intervention and control groups will be carried out using block randomization. To achieve this, four sheets of paper will be prepared - two with the name of the intervention and two with the name of the control. These paper sheets will be pooled and placed in a container. Patients will be selected one at a time without replacement, and for each patient, a paper sheet will be randomly drawn from the container. After each draw, the paper sheets will be returned to the container, and the process will be repeated until the desired sample size is reached.

Description
Intervention group: Routine treatment plus self-stretching exercises at home for 30 seconds 4 times a day for 6 weeks (the patient should stand against a wall or a vertical surface at a right angle with the body completely parallel to the vertical surface while the elbow is fully extended, abduct the upper body straight and extended in front of the trunk so that the angle between the arm and the trunk reaches 90 degrees. The distance between the patient and the opposite surface should be equal to the distance from the shoulder to the wrist, and the wrist should be extended to 90 degrees so that the palm and fingers are on a flat surface. Then the patient hyperextends the thumb and thenar muscles towards himself and repeats this for 30 seconds each time)

Description
Control group: Routine treatment plus sham stretching movement at home for 30 seconds 4 times a day for 6 weeks (sham stretching movement is completely ineffective in the treatment of carpal tunnel syndrome. The patient should hold the affected hand from the forearm in front of their face in supine position, then the non-affected hand should put pressure on the place where the nerve passes in the wrist with the thumb)
