Protocol summary

Study aim
Comparing the Impact of 'Visual Thinking Strategies' Training Based on the PRISM Model and the Training Method Using 'Observation Exercises' on the Observation Skills of Medical Students at Gonabad University of Medical Sciences
Design
The study includes control and intervention groups for training the observation skills of first-year medical students. It is a double-blind, non-randomized study. The sample size was calculated using a formula, resulting in 35 participants per group, which was increased to 42 per group to account for a 15% dropout rate.
Settings and conduct
The study is conducted at the medical school of Gonabad University of Medical Sciences. Evaluators and the statistician will not be aware of the intervention and control groups (double-blind study).
Participants/Inclusion and exclusion criteria
A medical student at Gonabad University of Medical Sciences, currently studying in the first year, willing to participate in the training course
Intervention groups
Initially, a pre-test will be conducted for all participants in both the control and intervention groups. Then, the control group will receive 'observation exercises' training over the course of one month. Subsequently, the educational intervention designed based on the PRISM model (using the 'Visual Thinking Strategies' method) will be implemented for the intervention group over one month. After the completion of the educational program, a post-test will be conducted for both groups. About a month later, to determine the application of the provided training in a real environment, an early clinical exposure session will be conducted for both the intervention and control group followed by another post-test.
Main outcome variables
Assessing the Observation Skills of Medical Students (number of observations, number of words used, time spent on analysis in descriptions, and type of observations)

Blinding description
Participants will not be aware of the type of intervention (single-blind study). During the evaluation phase, students' information is recorded anonymously using student codes, and the evaluator does not know which student code belongs to which individual or group. Similarly, during the analysis phase, the data analyst uses codes 1 and 2 assigned to each intervention and control group for analysis, without knowing which code corresponds to which group. (Double-blind study). All study participants will be asked to complete informed consent and a baseline survey through ePoll, an online survey management system.
