Settings and conduct
For 3 months, all telephone resuscitations in Hamedan province will be included in the control group, and after collecting the samples of the control group, the intervention will be done, and then for 3 months, the data will be collected after the intervention.
The intervention will be conducted in the amount of 3 sessions of 4 hours in the training workshop on the management and guidance of cardiopulmonary resuscitation based on the four-level triage algorithm approved by the Emergency Organization on experts working in telephone triage. The main steps of this model (T-CPR and V-CPR) are taught and the personnel are asked to write down and remember these items. In the next 2 sessions, how to use this model in simulated scenarios will be examined. In these 2 sessions, by using pre-prepared scenarios, the weak points of the personnel will be determined and simulated exercises will be conducted so that the personnel will be at the highest level of preparation for training. Telephone resuscitation (T-CPR) and video resuscitation (V-CPR).

Entry criteria: Level One: (Triage personnel) Willingness to participate in the research
Level two: (Calls recorded as cardiac arrest) calls recorded to the pre-hospital emergency room with complaints of cardiac and respiratory arrest, in order to control confounding variables such as the average time of arrival of the ambulance in both periods of this time in the range of 8 to 15 minutes will be included in the study, and if there is too much time in both periods, the sample will be excluded from the study, and when evaluating the results, the average time of ambulance arrival in both periods will be reported
Exclusion criteria:
Level one (triage personnel): Absence of more than one session in the training course, refusal to participate in the research

Description
This study is a pre-post clinical trial that will be conducted on personnel working in the pre-hospital emergency telephone triage unit of Hamadan province. For this purpose, all telephone resuscitations will be included in the control group for 3 months, and after collecting the control group samples, the intervention will be performed, and then the post-intervention data will be collected for 3 months.The intervention will be carried out in the form of 3 sessions of 4 hours in a training workshop on cardiopulmonary resuscitation management and guidance based on the four-level triage algorithm approved by the Emergency Organization on experts working in telephone triage. (The template is attached). In the first session, after introducing the participants to the purpose of the research and familiarizing them with the research steps, the main steps of this template (T-CPR and V-CPR) will be taught, and the personnel will be asked to write down and remember these items. In the next two sessions, the application of this model in simulated scenarios will be examined. In these two sessions, using pre-prepared scenarios, the weaknesses of the personnel will be identified and simulated exercises will be conducted so that the personnel will reach the highest level of readiness for telephone resuscitation (T-CPR) and video resuscitation (V-CPR) training. In telephone resuscitation, an attempt is made to train the bystander through telephone consultation and convince them to start chest compressions, but in video training, an attempt is made to prepare the bystander for this action by establishing a video call or sending a short clip (less than one minute) of how to perform chest compressions. In both methods, triage personnel try to teach the bystander to correctly place the heel of the hand on the chest, the number of massages and the depth of the massage, and continue the quality of the massage with effective encouragement until the arrival of emergency technicians. Then, in the real environment, the performance of telephone triage experts (outcomes of cardiopulmonary resuscitation) is evaluated over a period of 3 months by reviewing the recorded sounds and the electronic file (PCR) in the pre-hospital emergency room and the file of patients transferred to the hospital emergency room compared to the 3 months before the intervention. It is worth noting that all operational technicians are told that in missions with complaints of cardiac and respiratory arrest, if BLS resuscitation is started by a companion or bystander, it should be noted on the PCR form. Participants are also emphasized that accompanying and encouraging the bystander by telephone to perform correct and quality massage, indicating the depth and location of the massage, until the arrival of emergency personnel is one of their main duties. During this accompaniment, triage personnel will notice whether the bystander (patient's companion) is continuing to perform the massage.
