Objective: To evaluate and compare the therapeutic effect of sildenafil gel, oral sildenafil pill and ozone therapy in patients with erectile dysfunction design: a randomized, single-blind, placebo-controlled, phase 3 clinical trial study population: erectile dysfunction, age between 30 and 70 years exclusion criteria: patients with penile anatomical problems, other sexual disorders, heart attack in the past six months, stroke, treated with inorganic nitrates interventions and outcomes: use of ozone therapy in four ways simultaneously Autohemotherapy, injection of ozone in the penis, ozone injection in the lumbosacral region and topical gel (ozolive). In One group the topical sildenafil gel half an hour before sexual contact and in another group oral sildenafil one hour before sexual contact and In placebo group we will use placebo pill and placebo gel before sex. For comparison of erectile function in patients between the four groups, the international index of the erectile function will be used. All patients before and after the study will be evaluated by questionnaire. Sexual Partner satisfaction questionnaire before and after the study to be completed by the patient's sexual partner. For All patients at baseline and after two weeks of taking the drug are penile arteries color Doppler ultrasound will be done. مداخلات و پیامدها: ازون تراپی به چهار روش بطور همزمان استفاده شامل اتوهموتراپی، تزریق ازن داخل آلت تناسلی مردانه ، تزریق پاراورتبرال در ناحیه لومبوساکرال و ژل موضعی ازن (ازولیو).در گروه ژل سیلدنافیل ، ژل سیلدنافیل موضعی نیم ساعت پیش از تماس جنسی و گروه قرص خوراکی یک ساعت پیش از تماس جنسی و گروه پلاسبو یک قرص پلاسبو و ژل موضعی پلاسبو پیش از تماس جنسی مصرف خواهند کرد.

بیماران در سه گروه درمانی تقسیم بندی می گردند.در گروه ژل سیلدنافیل ، ژل سیلدنافیل موضعی نیم ساعت پیش از تماس جنسی به آلت تناسلی مردانه زده می شود. قرص پلاسبو نیز به بیمار داده می شود که یک ساعت پیش از تماس جنسی مصرف نماید.

بیماران بصورت دسته بندی شده (blocked randomization) و اختصاص گروه درمانی با استفاده از نرم افزار اکسل و استفاده از فانکشن Rand صورت می گیرد.گروه بندی بیماران بصورت گروه های 4 نفره در نظر گرفته شد. درمانگر و بیماران از نوع درمان اطلاع ندارند.

Ozonized olive oil (the amount of a teaspoon twice a day) is applied on the penis by the patient. (Intervention group)

ازون تراپی ابا چهار روش بطور همزمان شامل اتوهموتراپی، تزریق ازن داخل آلت تناسلی مردانه ، تزریق پاراورتبرال در ناحیه لومبوساکرال و ژل موضعی ازن (ازولیو). جهت اتوهموتراپی 50میلی‌لیتر از خون خود بیمار توسط سرنگ‌های استریل یکبار مصرف خارج و همراه با میزان مشخصی اُزُن ترکیب می‌شوند. روغن زیتون اوزون‌دار شده به میزان یک قاشق چای خوری دو بار در روز بر روی آلت تناسلی مردانه توسط خود بیمار مالیده می شود.( در گروه مداخله)

1- Evaluation of the effect of sildenafil gel on the improvement of erectile dysfunction before and after treatment

Settings and conduct
In the present study, the physician and the drug provider will not be aware of the type of treatment and the group of patients. All patients will receive tablets and gels that have the same appearance and are not aware of the treatment group.
setting : Urology clinic of baqiatollah Hospital in Tehran and Ali Ebn Abitaleb Hospital in Zahedan
Participants/Inclusion and exclusion criteria
Entry criteria: Erectile dysfunction, age 30 to 70 years
Exit criteria: Patients with anatomical problems in the penis, other sexual disorders, spinal cord injuries, heart attack in the past six months, stroke, treatment with mineral nitrates, gastric ulcer, migraine, visual impairment and allergic rhinitis
Intervention groups
In the sildenafil gel group, the topical sildenafil gel was given. A placebo tablet is also given to the patient to take an hour before sexual contact.
In the oral pill group, an oral dose of sildenafil is consumed. Also, a placebo gel is placed on the male penis.
In a placebo group, patients will receive a placebo tablet and placebo gel before sexual contact.
Main outcome variables
Erection, increasing the IIEF, improving the parameters of the penis color Doppler ultrasonography after sexual stimulation, increasing spouse's satisfaction

Exclusion criteria:
Penile anatomic abnormalities
other sexual dysfunctions
spinal cord injuries
Acute myocardial infarction within last 6 months
Cerebrovascular accident
nitrate drug use
Peptic ulcer disease
Migrane
visual disturbances

Randomization description
All patients will be randomized with a blocked randomization technique. The treatment group will be assigned with Excel software and use of Rand Function. Patients will be grouped into four-person groups. Physician and patients are not informed about treatment.

In the present study, the physician and the drug provider will not be aware of the type of treatment and the group of patients. All patients will receive tablets and gels that have the same appearance and are not aware of the treatment group. Medications with labels A and B are indicated for final analysis and return of results.
